PURSUANT TO PUBLIC ACT 08-167

BE ADVISED:

This business collects personal information in the course of business.

"Personal information" means information capable of being associated with a particular individual through one or more identifiers, including, but not limited to, a Social Security number, a driver's license number, a state identification card number, an account number, a credit or debit card number, a passport number, an alien registration number or a health insurance identification number, and does not include publicly available information that is lawfully made available to the general public from federal, state or local government records or widely distributed media.

IT IS THE POLICY OF CODY & GONILLO, LLP TO PROTECT THE CONFIDENTIALITY OF YOUR PERSONAL INFORMATION. 

IT IS AGAINST COMPANY POLICY TO DISCLOSE YOUR SOCIAL SECURITY NUMBER WITHOUT YOUR WRITTEN CONSENT.  DISCLOSURE OF SOCIAL SECURITY NUMBERS SHALL BE LIMITED TO THIRD PARTIES FOR WHOM YOU HAVE GIVEN EXPRESS WRITTEN CONSENT, OR TO THIRD PARTIES WITH WHOM YOU HAVE, ON FILE, A SIGNED HIPPAA CONFIDENTIALITY AGREEMENT.

ACCESS TO THE PERSONAL INFORMATION COLLECTED BY THIS BUSINESS IS STRICTLY LIMITED WITHIN THE BUSINESS TO MEMBERS AND EMPLOYEES OF THE LLP.

WE DO NOT DISCLOSE ANY NONPUBLIC
PERSONAL INFORMATION ABOUT YOU
TO ANYONE FOR ANY PURPOSE THAT IS NOT
SPECIFICALLY PERMITTED BY LAW.

We make reasonable efforts to restrict access to nonpubli
personal information about you to those employees who need
to know that information in order to provide services to you.
We also make reasonable efforts to train those employees to
protect the privacy of our clients.  We maintain physical,
electronic, and procedural safeguards that comply with federal
and state law to guard your nonpublic personal information.

 It is the policy of this firm to:

  • Protect the confidentiality of Social Security numbers and nonpublic personal information;
  • Prohibit the unlawful disclosure of Social Security numbers and nonpublic personal information; and
  • Limit access to Social Security numbers and nonpublic personal information.
